Services:


Contacting Your Doctor
The doctor can be contacted during surgery hours.
When the doctor is consulting with another patient a message will be taken and the doctor asked to ring you back.
However, emergency calls will be dealt with immediately.
15 minutes is allowed for most appointments. If more time is needed for consultation with the doctor please advise the receptionist when you make your appointment.
Interpreter Service can be accessed.

Fees: Bulk Billing.
However, bulk billing does not apply for Workcover, TAC and certain medicals.

Home Visits:
Are available to regular patients of the practice, who are unable to attend the surgery because of their medical condition. Visits can be arranged by telephoning your regular surgery/medical practitioner.
Visits can also be arranged for patients in nursing homes and hospitals.

Surveys:
From time to time, Dunmunkle Health Services invites patients to complete a questionnaire on the views of the practice. These surveys are confidential and help to improve the services offered to you.

Patient Medical History:
The patient’s medical history remains the property of Dunmunkle Health Services. However, when a patient wishes to consult with another practitioner outside of Dunmunkle Health Services, Dunmunkle Health Services requires a letter of Request for Release of Medical History from the chosen doctor. Dunmunkle Health Services will then post the information to the requesting doctor.

Recall System:
Dunmunkle Health Services medical practices use a manual and computerised recall system. If you do not want to be a part of the system, let the receptionist or your doctor know. A reminder phone call or letter will be sent before the due date.

Prescriptions:
All patients are required to make an appointment with their doctor when a prescription expires. Occasionally, the doctor will write a script to "tide you over". However, this is not common practice and you must visit your doctor as soon as possible following this.

Privacy:
Dunmunkle Health Services maintains the privacy/confidentiality of the client's personal information in accordance with the Health Records Act 2001 (Vic) and Information Privacy Act 2000 (Vic). Individuals have the right to access and correct personal health information through the Freedom of Information Act 1982. For further information a pamphlet entitled Protection & Use of Your Health Information is available.

Problems/Concerns/Complaints:
If you have a problem with our service we would like to hear about it. Please feel free to talk to the doctor or receptionist. You may prefer to write to us or complete a Complaints form which can be obtained at each facility. We take your concerns, suggestions and complaints seriously.

Consumer Complaints:
Complaints to be in writing. (A Complaints form is available, on request, from any Dunmunkle Health Services facility). To be addressed to either the campus Manager or the
